Discuss employment and education history. How might having this information about the client impact your work together in treatment?

Theory ModelANDONEIndividual Theory Model(that appears from the list below), and a treatment plan that reflects theoretical orientation and goals for the client. The assignment is your opportunity to critically evaluate the role of theory in conceptualization and practice. It is expected that you demonstrate diversity and cultural awareness as it applies to the individual cases. Treatment plans should include an overview of the case, a brief presentation of the theory, and a client specific plan for a beginning, middle and end of treatment with appropriate interventions. Students should offer a few examples of these interventions. In addition, it is expected that students will make use of assigned texts and other materials for references. A sample of what the professor is expecting is listed below. Please review the template below. Do not alter the main template headings. You are required to use this template for this assignment.

The Case Conceptualization Paper is designed to have the student select one Individual Theory and one Family Theoryof the following theories and apply these two theories to the critical review of the vignette below using the template below:

  • Freud’s Psychoanalytic Theory
  • Adler’s Psychodynamic Theory
  • Existential Theory
  • Gestalt Theory
  • Rogers Person Centered Theory
  • Cognitive Behavior Theory
  • Behavior Theory
  • Bowen’s Family Theory
  • Strategic Family Therapy

-Structural Family Therapy

Students will use the vignette below for this project:

Imagine that you are working with the following client in therapy

Mike is a 19 year old African American male (he has two brothers and he is the youngest child). His parents brought him to therapy because Mike has been reportedly stressed. He is reported to worry for prolonged periods of time. His worrying has interfered with his grades in school. He can’t focus and concentrate and he is failing several classes. His sleep has decreased because he worries about various issues (no particular topic). In therapy he reported that his symptoms of excessive worrying began about one year ago. He states, “I feel like I can’t go anywhere alone. I feel like I always have to go somewhere with someone. I feel like I depend on others too much but it’s my reality. It’s like I need someone to protect me. I have lost relationships with good friends because they tell me that I am too needy.” Mike mentions that if he goes places alone he starts to feel stressed and he describes the following symptoms: “I feel nervous and scared, like something bad is going to happen, I feel my heart racing, my muscles get real tense and I get dizzy and nausea.” “I feel like I should just stay home and avoid going anywhere but I have to graduate high school.” Mike reports he was working a part time job that he eventually quit because he kept calling in sick and avoiding work. His mom mentioned that now he is trying to avoid going to school and she is afraid he may not graduate. In the session he makes the following statements: “If I go places alone, than I feel like something bad is going to happen.” “If I don’t get my act together, I know my life is over. Why does everything feel like it’s a catastrophe.” Mike’s mom reported that he recently began drinking. Mike states, “I drink because it makes me feel better, it makes the stress decrease and the worrying decrease for awhile.”

Introduction to the Theoretical Model

In this section summarize the previous ONE Individual Theory AND ONE Family Theory model that you selected from the list that was indicated above. Describe the main points of these theories, using the language of these theories alone (don’t borrow concepts from other theories, stay focused on the one Individual Theory and one Family Theory you select and be like an expert). This section should be a minimum of a one page and no more than 3 pages (one and a half pages to describe the first Individual Theory and one and a half pages to describe the second Family theory you selected.

History of the Present Illness

A detailed chronological description of problems and concerns experienced by the client, including onset, frequency, intensity, and duration of symptoms.

DSM V Diagnosis(including use of V-Codes)(list the diagnosis and rationale / should match symptom presentation listed above). Take this information straight from the DSM V and list criteria related to the diagnosis you selected.

How might this information impact diagnosis, treatment and creation of treatment goals?

Assessment

Whatare specific assessment measures would you decide to conduct to enhance your understanding of the client(s) in order to direct your diagnostic formulation? Do a literature review and or research current measures that assess the symptoms of the diagnosis you gave the person.

Theoretical Formulation: Family Theory/Therapy (Case conceptualization)

  1. Name a specific family theory you would use to treat this case and why you would choose this theory.
  2. Describe the specific and fundamental tenets of your chosen family theory.
  3. Apply the family theory to your case, specifying hypotheses based on your theory.

Theoretical Formulation: Individual Theory/Therapy (Case conceptualization)

Note: You must use a separate theory from the family theory section.

  1. Name a specific individual theory you would use to treat this case and why you would choose this theory.
  2. Describe the specific, fundamental tenets of the individual theory.
  3. Apply the individual theory to your case, specifying hypotheses based on your theory.
